Deductible Choices - $250 to $2,000

Deductible Credit - At plan inception, participants can transfer deductible credits from previous plan.

Coinsurance Choices - 90/70, 90/60, 80/60, 80/50

Comprehensive Office Visit Benefit - Most plans include copay benefits under one of the broadest office visit definitions available.

Wellness Benefit - Coverage for preventive care services through Preferred Provider Physicians.

LabOne - Outpatient lab test processed by LabOne are covered in full, not subject to copays, deductibles or coinsurance on all PPO plans.

Prescription Drug Card - Depending on Plan selected, either a copay drug benefit or discount card is included.

"Multiple Choice" Benefit Option - Employees can choose between 2 different benefit plans selected by the employer when at least five employees have medical coverage.

Emergency Treatment - Even if this care is received outside of the PPO network, it is covered at in-network benefit levels.

Out-of-Area PPO Access - When medical care is required outside of your PPO's service area, insured have access to on of the largest national PPO networks available.

Transplant Benefit - Most organ transplants are covered as any other illness up to the policy limits when performed by a Preferred Center of Excellence Transplant Provider.

Express Health Line - Toll-free access to professional healthcare information and counseling is available 24 hours a day.

Discount Eyecare - A nationwide network of eyecare specialists offers substantial discounts on eyewear and eyecare services.

Section (125) Premium Only Plan (POP) - All the material and assistance needed to implement a Premium Only Section (125) plan is included with all of our medical plans.
